Customer Feedback Forms



It is a requirement of the ISO9001:2015 Quality Management system audited annually by BSI that we obtain regular feedback from our customer base, be it good, not so good or bad ....we need to know! 

We have designed two forms to capture feedback concerning our products, the first for non clinicians and the second taylored to a particular product group for clinicians.

1.o Standard Customer Feedback Form for non-clinicians to complete, comprising 10 MCQ concerning 3 subjects, the procurement route, product performance and company performance respectively.

2.0 Clinical Feedback Supplementary Form for clinicians to complete, comprising circa 15 questions with a more clinical focus specific to each of the following product groups:
